Slight changes to explanations of maths elements.
Noted that entity references should not be used inside maths elements.
Substantial edit. Changes to shortref cleverness made some of the advice in here wrong. It now tells folk not to use entity references at all in maths.

Added sgml2docs command. Significant changes to programcode. Incorporates decisions from Durham, RAL and Cambridge SGML meetings. This is an interim release, intended as a snapshot for distribution to Rodney Warren-Smith and Mark Taylor. There will be a proper distribution before 1 June.
Noted that the stylesheets deal gracefully with missing FPIs
Principal table attributes now supported in LaTeX down-converter.

Added a note pointing out that returnvalue and argumentlist have optional `none' attributes.
Returnvalue is now omissable
none. The argumentlist
element is not omissable, however (since if the SGML parser finds a
parameter element, it can't know whether it's part of an argumentlist
or a parameterlist element), and this also takes an attribute none.Paragraph rewritten to suggest using a CDATA marked section, instead of code edits.
This section trimmed, and the discussion of the `author-visible' uses of MSs moved to the `Marking up...' section.
Very substantial additions - this section was barely present before.
